ECF CHOICES Program. If Provider renders Covered Services to Covered Persons under TennCare’s program for people with Intellectual and/or Developmental Disabilities, Provider shall facilitate notification of the Covered Person’s Support Coordinator by notifying Health Plan and/or Subcontractor, in accordance with Health Plan’s and/or Subcontractor’s processes, as expeditiously as warranted by the Covered Person’s circumstances, of any known significant changes in the Covered Person’s condition or care, hospitalizations, or recommendations for additional services.
