Elected Benefits. Arrangements regarding an employee’s payment of dependent medical/dental insurance premiums and/or other voluntary deductions, if continuity is desired, must be made by the employee with the Human Resources Department prior to starting any unpaid leave of absence.
Elected Benefits. Arrangements regarding employee’s payment of dependent medical/dental insurance premiums and/or to his or her voluntary deductions, if continuity is desired, must be made with Human Resources prior to starting LWOP. The failure of the employee to make the payment shall result in cancellation of the coverage.
