Evaluation Report. A. The administrator/principal or immediate supervisor will review the Evaluation Report Form for the employee's personnel file at the conference. The final evaluation conference shall be held no later than five (5) school days following the final observation of the employee. B. The employee's signature on the Evaluation Report Form shall not necessarily indicate concurrence with the evaluation. The evaluee has the exclusive right to attach an addendum statement. C. Distribution of the final Evaluation Report Form shall be: 1. original - school district personnel file; 2. first copy - principal; and 3. second copy - employee
