Common use of Evidence of Coverage Clause in Contracts

Evidence of Coverage. The Contractor shall submit Certificates of Insurance to DSHS for each coverage required of the Contractor under the Contract. The Contractor shall submit the Certificates of Coverage to Central Contract Services, Post Office Box 45811, Olympia, Washington 98504-5811 or XXXXxxxxxxxxXxxxxxx@xxxx.xx.xxx. Each Certificate of Insurance shall be executed by a duly authorized representative of each insurer, showing compliance with the insurance requirements specified in this Contract.
