Exclusive Use of Health Centers by Eligible Persons. Only Eligible Persons shall be permitted to obtain services at the Health Centers. The Employer shall use commercially reasonable efforts to ensure that the Census provided to LGMG contains accurate information regarding the eligibility status of each individual listed. Prior to providing services to an individual at the Health Centers, LGMG shall request a form of identification from the individual and then check the most recently received Census to determine whether the individual is an Eligible Person. LGMG shall direct any questions regarding an individual’s eligibility to receive services at the Health Centers as an Eligible Person to the Employer’s Plan Representative or the Employer’s Plan Representative’s designee.
