Final Expenditures Report. The Project Completion and Certification Report (P4) must be submitted to TDEM within 90 days of all approved work being completed for each project. If any project requires the purchase of insurance as a condition of receiving federal funds, a copy of the current policy must be attached to this report and the Applicant’s Attestation for Duplication of Benefits (DOB) form certifying other funds were received to complete the project.
