Common use of For all Children Covered as Adopted Children Clause in Contracts

For all Children Covered as Adopted Children. Coverage for an adopted child will not be required if the child is not ultimately placed in the Contractholder’s home in compliance with Chapter 63, Florida Statutes. If the final decree of adoption is not issued, coverage will not be continued for the proposed adopted child. Proof of final adoption must be submitted to us. It is your responsibility to notify us if the adoption does not take place. Upon receipt of this notification, we will terminate the coverage of the child on the first billing date following receipt of the written notice.
