Dependent Enrollment. All staff will have the option of having their dependents attend the school where they teach. If the staff member lives out of district, the tuition application process must be completed. If the student’s grade level is not consistent for that school, the student will be allowed to attend the nearest school to the staff member’s employment. If the staff member wishes to have their student attend a school other than the one at which they teach, the transfer process must be completed and the placement will be at a mutually agreed upon site. In the event of disagreement, final placement will be at the discretion of the superintendent. Article 15.9 will not be subject to the grievance procedure or arbitration. Staff will make annual requests in writing to the building principal or the enrollment office on or before April 30th. The student may remain in the district in accordance with the transfer and tuition policies.

Dependent Enrollment. Any full-time member of the bargaining unit with dependents enrolled in the District shall have the right to determine in which District school(s) those dependents shall be enrolled. This enrollment shall depend on the availability of space in that school once the District’s normal registration processes have been followed. Requests shall be directed to the attention of the Assistant to the Superintendent.
Dependent Enrollment. If electing dependent coverage, the employee must enroll eligible dependents, as agreed to by the policyholder and us, within 31 days of the dependent's eligibility date or within the time period specified in the "Special enrollment" provision. The dependent is a late applicant if enrollment is requested more than 31 days after the dependent's eligibility date or later than the time period specified in the "Special enrollment" provision. A late applicant must wait to enroll for coverage during the open enrollment period, unless the late applicant becomes eligible for special enrollment as specified in the "Special enrollment" provision. Health status will not be used to determine premium rates. We will not use health status-related factors to decline coverage to an eligible dependent and we will administer this provision in a non-discriminatory manner.

  • Re-enrollment Any eligible employees who wish to join the Sick Leave Bank after their first year of eligibility will contribute two (2) days upon joining. Such membership may only be made during the month of October using the appropriate forms. The two (2) required days of leave shall be donated from their account upon enrollment in the Classified Employee Council (CEC).

  • Enrollment The School shall maintain accurate and complete enrollment data and daily records of student attendance.

  • Medicaid Enrollment Treatment Grantees shall enroll as a provider with Texas Medicaid and Healthcare Partnership (TMHP) and all Medicaid Managed Care organizations in Grantee’s service region within the first quarter of this procurement term and maintain through the procurement term.

  • Canceling Dependent Coverage During Open Enrollment In addition to the above situations, dependent health or dependent dental coverage may also be cancelled for any reason during the open enrollment period that applies to each type of plan (as long as allowed under the applicable provisions, regulations and rules of the federal and state law in effect at the beginning of the plan year).

  • Special Enrollment Period An eligible individual and eligible dependents may be enrolled during special enrollment periods. A special enrollment period may apply when an eligible individual or eligible dependent loses other health coverage or when an eligible individual acquires a new eligible dependent through marriage, birth, adoption or placement for adoption.
