FOR PERSONAL ILLNESS OR INJURY. (i) An Employee will, upon request, provide evidence to the Employer for each absence of two consecutive days or more due to personal illness or injury. (ii) Evidence may be obtained from either a medical practitioner or from a registered health practitioner. In accordance with the Health Practitioner Regulation National Law (NSW) or Health Practitioner Regulation National Law (ACT) a registered health practitioner means an individual who practises one of the following professions including its specialities: Chiropractic Dental (including the profession of a dentist, dental therapist, dental hygienist, dental prosthetist and oral health specialist) Medical Nursing and midwifery Optometry Osteopathy Pharmacy Physiotherapy Podiatry Psychology Aboriginal and Xxxxxx Xxxxxx Islander health practice Chinese medicine Medical radiation practice; or Occupational therapy.
