Common use of Form of Evaluation Clause in Contracts

Form of Evaluation. Performance evaluations are expected to conducted using the form prescribed by the Human Resources Department. Where an employee has worked under several supervisors, each supervisor should contribute to the evaluation. Each performance evaluation shall be discussed with the employee. The employee shall sign the report as a means of acknowledging its content. Such signature shall not necessarily mean the employee agrees in total with the contents of the evaluation report. After the evaluation has been completed and signed, a copy shall be given to the employee and the supervisor. In addition, a copy is to be filed with the Human Resources Department.
