Common use of GROUP HEALTH & LIFE INSURANCE Clause in Contracts

GROUP HEALTH & LIFE INSURANCE. For those bargaining unit members employed on the date of ratification of this Agreement, group health and life insurance is carried for permanent employees and their dependents, if the employee works a minimum of fifteen (15) hours per week. For those bargaining unit members employed subsequent to the date of ratification, the minimum of fifteen (15) hours per week changes to twenty (20) hours per week. Employees who work less than twelve (12) months a year will also be covered when not on duty status, limited to 3 months per year when school is not in session. The Board shall provide at a premium cost to the eligible employee of $1080 per year payable through payroll deductions, health insurance coverage for the eligible employee and the eligible employee’s spouse and eligible dependents in accordance with the plan described in part in the attached document entitled Northwest Arctic Borough School District Health Care Plan, Group No. 12431, Summary Plan Description, including the Health Care Review Service. The premium payment shall be retroactive to July 1, 2017, for those bargaining unit members employed on March 27, 2018. The District has negotiated a steerage preferred provider/network plan for hospitalization (in-patient and out- patient) and service providers The penalty co-payment shall be 40 percent for Out of Network hospitalization, thereby reducing the District’s co-payment from 90% to 50%. The penalty co-payment shall be 20% for Out of Network service providers, thereby reducing the District’s co-payment from 90% to 70%. An eligible employee who chooses not to pay that yearly premium cost shall waive the Board provided health insurance coverage, including such coverage as a spouse or dependent. Said premiums shall be eligible for the District’s Section 125 plan. Coverage shall begin on the first day of the month coinciding with, or following, the eligible employee’s first day of work for the District. Coverage shall terminate on the last day of the month coinciding with, or following, the eligible employee’s last day of work for the District. Effective December 1, 2018, the Plan’s calendar year deductible increases to $100/$300 single/ family In Network; $600/$1800 single/ family Out of Network; Out-of-Pocket Maximums increase to $900/$2000 single/family In Network; $3000/$6000 single/family Out of Network. Effective December 1, 2019, the Plan’s calendar year deductible increases to $150/$450 single/family In Network. Out-of Pocket Maximums increase to $1000/$2750 single/family In Network.
