Group Insurance Plans. Group insurance benefits are provided to employees based on their classification and work schedule. Employees who are Regular Part or Full time are eligible for the myBenefits modular plan; plan eligibility varies based on scheduled hours of work. Employees who are term or short term are eligible for option 1 (Base Plan) for Medical and Dental and other benefits per duration of term assignment. Group Insurance benefits and payment sharing outlined below are set out in the 2017 MyBenefits Booklet. For clarity, benefit entitlements and cost sharing will not be altered without SPEA’s agreement and the group insurance provider may be altered at the Company’s sole discretion.
Group Insurance Plans. Not later than the Distribution Date, Healthcare shall establish the Healthcare Group Insurance Plans and Electronics shall establish the Electronics Group Insurance Plans effective as of the Distribution Date and, correspondingly, Healthcare Employees and Electronics Employees shall cease participating in the Tyco Group Insurance Plans on the dates the new plans are established and shall begin participating in the Healthcare Group Insurance Plans or the Electronics Group Insurance Plans, as applicable. After the Distribution Date: (i) Healthcare shall be solely responsible for the administration of the Healthcare Group Insurance Plans and solely responsible for the payment of all employer-related costs in establishing and maintaining the Healthcare Group Insurance Plans, (ii) Electronics shall be solely responsible for the administration of the Electronics Group Insurance Plans and solely responsible for the payment of all employer-related costs in establishing and maintaining the Electronics Group Insurance Plans, and (iii) Tyco shall retain sole responsibility for all benefit obligations under the Tyco Group Insurance Plans and shall be solely responsible for the payment of all employer-related costs in maintaining the Tyco Group Insurance Plans.
Group Insurance Plans. Trident shall cause Fountain to establish the Fountain Group Insurance Plans, effective no later than the Fountain Distribution Date and, correspondingly, except as provided below, Fountain Employees shall cease participating in the Trident Group Insurance Plans on the dates the new plans are established and shall begin participating in the Fountain Group Insurance Plans. The newly established Fountain Group Insurance Plans shall be substantially similar to the Trident Group Insurance Plans. After the Fountain Distribution Date: (i) Fountain shall be solely responsible for the management and administration of the Fountain Group Insurance Plans and solely responsible for the payment of all employer-related costs in establishing and maintaining the Fountain Group Insurance Plans, and (ii) Trident shall retain sole responsibility for all Liabilities for claims incurred prior to the Fountain Distribution Date under the Trident Group Insurance Plans and shall be solely responsible for the payment of all employer-related costs in maintaining the Trident Group Insurance Plans.
Group Insurance Plans. 15.01 The Company agrees to provide all full time employees with one (1) or more years of continuous service, a *Short Term Disability Benefit (S.T.D.) as generally described in Section B of a notice to all employees of the amended Benefit Plan dated May 1, 1993. *Payment for Short Term Disability shall begin on the third (3rd) consecutive day of absence. For those employees who have completed ninety (90) days of perfect attendance from the last date of return to work from an absence due to sickness or accident, shall be paid S.T.D. from the first (1st) day for the first covered absence following the ninety (90) days of perfect attendance. First entitlement to paid S.T.D. shall not occur before April 1, 2003. For the first ninety (90) days of 2003, the Company will recognize those employees who have completed one
(1) year of perfect attendance from the previous year, and those employees shall be paid

Tyco shall cause ADT NA to establish the ADT NA Group Insurance Plans, effective no later than the ADT NA Distribution Date and, correspondingly, except as provided below, ADT North American R/SB Employees shall cease participating in the Tyco Group Insurance Plans on the dates the new plans are established and shall begin participating in the ADT NA Group Insurance Plans. The newly established ADT NA Group Insurance Plans shall be substantially similar to the Tyco Group Insurance Plans. After the ADT NA Distribution Date: (i) ADT NA shall be solely responsible for the management and administration of the ADT NA Group Insurance Plans and solely responsible for the payment of all employer-related costs in establishing and maintaining the ADT NA Group Insurance Plans, and (ii) Tyco shall retain sole responsibility for all Liabilities for claims incurred prior to the ADT NA Distribution Date under the Tyco Group Insurance Plans and shall be solely responsible for the payment of all employer-related costs in maintaining the Tyco Group Insurance Plans.
Group Insurance Plans. 32.1 The Employer shall provide employees with the following benefits, and it is recognized that they may enter into agreements with insurance underwriters in order to do so:
1. A Supplementary Life Insurance Plan.
2. A Dependents’ Life Insurance Plan.
3. A Basic Life Insurance Plan.
4. A Long Term Income Protection Plan.
5. A Supplementary Health and Hospital Insurance Plan.
6. A Dental Insurance Plan.
32.2 Employees will be insured for Supplementary and Dependent Life (when elected), Basic Life, Long Term Income Protection, Supplementary Health and Hospital benefits, and the Dental Plan effective the first day of the month immediately following two (2) months continuous service.
a) During leaves-of-absence with pay including vacation, WSIB, and short- term sickness, full benefit coverage will continue. The Employer will continue to pay the premiums otherwise payable by the Employer for the group insurance coverage and the employee will continue to pay the premiums for the group insurance coverage that the employee was paying immediately before the leave.
b) During pregnancy and parental leave, an employee who participates in the group insurance coverage related to his / her service with the Employer, may continue that participation unless he / she elects in writing not to do so. Unless an employee gives the Employer this written notice, the Employer shall continue to pay the premiums for the group insurance coverage that the Employer was paying immediately before the employee’s pregnancy leave or parental leave and the employee shall continue to pay the premiums for the group insurance coverage that the employee was paying immediately before the leave.
32.4 During leaves-of-absence without pay of less than one (1) calendar month, full benefits coverage will continue.
32.5 Except as provided in Articles 32.3(a), 32.3(b) and 32.4 above, the group insurance coverage referred to in Article 32.1 shall not be provided for an employee during a leave of absence without pay except to the extent that the employee arranges through the payroll or personnel branch of his / her Ministry to pay the amount of the full premium for any of the coverages that the employee chooses to have continued during the leave and pays the amount at least one week before the first of each month during the leave of absence.
32.6 Within a reasonable time after granting a leave-of-absence without pay to an employee, the Employer shall inform the employee that group insurance coverages duri...
Group Insurance Plans. The parties agree that should the Company increase the plan through bargaining with the hourly employees or in discussions with the Union at any time, then such increase will be provided to the Dependent Contractor Plan as well. The Company agrees that should the Union wish to move the Dependent Contractor plan to another carrier or provider they shall be entitled to do so with a minimum of thirty (30) days notice. The Company agrees to provide any necessary figures or statistics to help the Union find costing for another carrier or provider.
12.1 a) The Company will provide Dependent Contractors and their dependents, excluding probationary Dependent Contractors, with the opportunity to participate in its Group Insurance Plan, including the following benefits (as defined below and part [d] of this article). The cost of the benefits below, including any and all premiums, shall be paid as per Article 12.1 (f):

From and after the Commencement Date, the Executive shall be entitled to participate in the Company’s group medical, dental, long term disability and short term disability insurance programs on the same terms and conditions applicable to the other senior officer(s) of the Company.
