Health Care Home Care Coordination Payment. Beginning July 1, 2010, the MCO shall pay a care coordination fee for qualified Enrollees of a certified Health Care Home within the MCO Provider network. The fee schedule must be stratified according to the stratification criteria developed by the STATE, pursuant to Minnesota Statutes § 256B.0753.
