Health Programs. 109. State Parties shall operate and maintain funding at current levels for the programs listed below, until the dates listed below, subject to applicable federal laws, necessary approvals from the Centers for Medicare & Medicaid Services, and availability of necessary federal monies:
a. The Medicaid expansion for pregnant women and children under twenty-one years of age up to 400% of the federal poverty level (as defined by the U.S. Department of Health and Human Services), through March 2021;
b. Elevated Blood Lead Level Case Management services for children with elevated blood lead levels, through September 2018;
c. Family Supports Coordination target case management services, through March 2021;
d. The Michigan Child Collaborate Care expansion, through September 2018;
e. The Child and Adolescent Health Center expansion, through September 2018;
f. The Michigan Nurse Professional Fund, through September 2018;
g. The Mobile Food Pantry expansion, through September 2018;
h. The Double Up Food Bucks expansion, through September 2018; and
i. The Summer Electronic Benefit Transfer for Children program expansion, through September 2018.
Health Programs. Immunization - a nurse, upon request, may receive immunization at the Hospital's expense.
Health Programs. Educational Support Series employees who are in the bargaining unit shall be covered by the same health insurance benefits as specified in Article 20.
Health Programs. A. The Township shall maintain all present State Health Benefit Plan hospital and medical insurance programs to which Employees are presently beneficiaries, including dependent coverage. The cost of coverage for the employee and dependents will be in accordance with state law. The Township’s base plan is NJDIRECT15. If an Employee elects coverage other than NJDIRECT15, he/she shall pay all the costs above the cost of NJDIRECT15 associated with the coverage elected. The maximum for chiropractic services shall be One Thousand Dollars ($1,000.00).
B. The Township shall continually investigate a more comprehensive group hospital and medical insurance program and, if such a program is adopted for other Township employees, it shall automatically apply to Township Superior Officers, provided substantially equivalent benefits and coverages are contained.
Health Programs. Complete physical examinations will be available to all unit employees at least once every three years.
Health Programs. 24.1 The Duluth Clinic will adhere to applicable guidelines it requires at no cost to the employee.
Health Programs. The Sheriff will comply with Public Act 57 of 1997 (MCL333.5204) HIV HBV Testing, Tuberculin Testing and Influenza Immunizations.
Health Programs. A. The Township shall maintain all present hospital and medical insurance programs to which Patrol Officers are presently beneficiaries. If opting for an HMO program, the Patrol Officer will pay the differential in premiums if higher than the standard plan. During times, if any, when the differential is less than the standard plan, the Patrol Officer will receive a credit bank against any obligation which occurs under the program from January 1, 1988 and thereafter. The maximum for chiropractic services shall be $1000.
B. If, for some reason, a Patrol Officer does not wish to be covered by said insurance programs, he shall not be entitled to receive the cash equivalent in lieu of the premium.
C. The Township shall continually investigate a more comprehensive group hospital and medical insurance program and, if such a program is adopted for other Township employees, it shall automatically apply to Township police officers, provided substantially equivalent benefits and coverages are contained.
D. The Township shall provide for complete physical examinations for several police officers each year, to be distributed on an age basis. Officers forty (40) years or older, every second year. Officers under forty (40) years every third year. The components to be included in the medical examination shall be reviewed by representatives of the Association and will be consistent for each examination conducted at a clinic to be arranged by the Township. Additional testing, examinations, or treatments recommended by this examination will be followed up by the officer under the provisions of the Medical Insurance Programs provided in this contract. Officers will follow recommendations of the doctors with respect to health habits. Employees must use the medical plan for their yearly physicals. If the plans do not cover examinations which are required by the Township, the Township is responsible for the cost.
E. The Township shall provide for each employee on a voucher reimbursement basis expenses incurred for eye exam, prescription lenses, and frames up to $200 every twenty-four (24) months.
F. Employees may opt out of the Township health coverage if the employee or employee's spouse has other health coverage. In lieu of receiving this coverage, the employees will receive $1000 in December of each year prorated for the number of months the employee is not covered under the Township health plan.
