Health Tests. At the time of employment, the Employer shall provide a Tuberculin skin test at no cost to the nurse. In the event of a positive reaction to this test, the Employer will provide a chest x-ray at no cost. Upon request, a routine blood examination and urinalysis will be provided at no cost to the nurse once each year.
Health Tests. The Employer shall bear the cost of and provide bargaining unit nurses with tuberculosis testing including chest x-rays where called for by state law, available on duty time at the University of Washington Medical Center or other designated facility. All nurses in the bargaining unit shall be permitted, upon request, a throat culture, the cost of which shall be borne by the Employer. When an RN feels at risk, the hepatitis B and C screen and hepatitis B vaccine, pre- and post- test HIV counseling and the HIV test will be offered at no cost with the approval of the Employee Health Nurse and the final determination by the Infection Control Committee. . Shingles vaccines (for RNs 60 years of age and over), DPT and Influenza vaccine will be offered to all RNs at no cost.
Health Tests. The bargaining unit member shall be given, upon request, a report of the examination and a confidential record shall be kept by the Employer. In addition, all CDL related examinations shall be done in accordance with any applicable laws.
Health Tests. The Employer follows CDC (Center for Disease Control and Prevention) recommendations and guidelines pertaining to TB skin tests for employees. Testing recommended by the above guidelines, or requested annually by the nurse, will be provided without cost to the nurse. Nurses will be screened for tuberculosis at hire and as needed for post- exposure monitoring. The Employer shall, at no cost to the nurse, provide a Hepatitis B series to any nurse requesting the series and make available follow-up testing to assess efficacy of the series.
Health Tests. Annually, the Hospital shall arrange for nurses to take Mantoux skin test and/or chest X ray, if indicated, at no cost to the nurse. All nurses shall be permitted a complete blood count, chest X ray, urinalysis, and pap smear annually when ordered by a physician at no cost to the nurse. Hepatitis testing and anti-hepatitis vaccine will be available at no cost to the nurse. If there is exposure to HIV, testing will be available at no cost to the nurse.

Health Tests. Nurses shall be entitled to routine blood examinations and urinalysis performed annually at the Employer’s hospital without cost.
Health Tests. At the time of employment and annually thereafter, all nurses shall receive a tuberculin skin test or chest X ray as required by the State of Washington. Upon written request, the appropriate vaccine will be made available if a nurse is identified as being at risk, based upon Infection Control Committee policies. If the nurse does not complete the entire series, the nurse will be responsible for reimbursing the Hospital for the cost of the vaccine.
Health Tests. If an employee has a positive tuberculin skin test result, the employee is required to complete a chest x- ray screening with a physician at the direction of the Employer and at no cost to the Employee.
