Hearing Aid Coverage for Children Program. a) Contractor shall provide information to Enrollees regarding the availability of the California Department of Healthcare Service’s (DHCS) Hearing Aid Coverage for Children Program (HACCP) within its Evidence of Coverage (EOC) and Disclosure Form. Information shall include notice that some Enrollees under age 21 in need of hearing aids and related benefits not covered in the QHP may be eligible for such benefits through the HACCP and provide contact information for the HACCP for Enrollees to find more information and apply for benefits.
