Home and Community-Based Services. (HCBS) Waiver HCBS waivers are excluded from the Hoosier Care Connect program. Individuals who have been approved for these waivers will be disenrolled from managed care. The Contractor shall coordinate care for its members that are transitioning into the waiver.
