Eligibility: After twelve (12) months of service with the Employer • Daily amount - the difference between the public xxxx rate and the semi- private accommodation rate • Benefit maximum age – retirement • Hospital co-insurance at 100% • Convalescent Care is semi private, limited to $20 per day to a max of 180 days • Dependent age coverage until 21 years
