Hospital Setting. The services provided by a Contracting Provider in a hospital setting are covered if the following criteria are met: 1. A hospital or similar setting is Medically Necessary. 2. The services are pre-authorized in writing by a Contracting Provider. 3. The services provided are the same services that would be provided in a dental office. 4. The Hospital Call Copayment and applicable Copayments are paid.
