Vision Care Plan Sample Clauses

Vision Care Plan. The County agrees to provide a Vision Care Plan for all employees and dependents. The Plan will be the Vision Service Plan - Plan A with benefits at 12/12/24 month intervals with twenty dollar ($20.00) deductible for examinations and twenty dollar ($20.00) deductible for materials. The County will fully pay the monthly premium for employee and dependents and pick up inflationary costs during the term of this agreement.

Vision Care Plan. The parties agree to the continuation of the Vision Care Plan with the following changes:
Vision Care Plan. The District will continue to provide a Vision Care Plan for employees, an employee’s eligible domestic partner (per Board Resolutions 4455 and 4457), and their eligible dependents providing the following: Visual Examinations 12 months Lens Replacement 12 months Frames Replacement 24 months There shall be a ten dollar ($10) deductible. The District shall pay the full premium for all employees and eligible dependents. Employees shall have the option to purchase, at their own expense, an enhanced Vision Care Plan. Benefits and premiums shall be established by the District annually based on plan experience. Premiums for this benefit will be paid through payroll deduction.
Vision Care Plan. The Company will provide a Vision Care Plan which will provide for expenses incurred by an employee and/or the employee's covered dependants when recommended by a physician or optometrist as follows: - frames, lenses, and the fitting of prescription glasses including contact lenses, up to a total of one hundred and eighty dollars ($180.00) per family member, in any two (2) consecutive calendar years.

Vision Care Plan. The Board will pay 100% of the premium cost for employees and their dependents for a vision care plan. The plan will pay a maximum of $150.00 towards the purchase of new or replacement eyeglasses, replacement parts of frames or replacement of lenses to existing eyeglasses or the purchase of contact lenses in lieu of eyeglasses or any combination thereof. The full benefit of $150.00 is available in a continuous 24-month period to commence from the date of most recent purchase made under the vision care plan.
Vision Care Plan. The Employer shall provide for a vision care plan to cover purchase and repair of glasses, as well as contact lenses to all full-time employees up to a cost not to exceed four hundred and seventy-five dollars ($475) which includes one eye examination within every 24 months with the ability to use coverage for laser surgery with the Employer contributing one hundred (100) percent of the cost of the premiums for this plan.

Vision Care Plan. The Board will provide single or family coverage vision care at no expense for all unit members enrolled in either the PPO-300, PPO-500, PPO750, or HSA-1500 health insurance plan. These benefits are considered outside the health insurance portion of the plans and are therefore not subject to the medical maximum benefit, deductible or out-of- pocket limit.
