Individual Records. Pharmacy must maintain an individual record of administration including: individual name, date, vaccine given, manufacturer, lot number, expiration date, and signature of person administering vaccine.
Individual Records. 10 Pharmacy must maintain an individual record including each vaccine administered. These records must include a copy of the written protocol, name and address of patient, age if under fourteen
Individual Records. The Engager shall ensure that a record is kept for each child, and the record must be available to the Child Supervisor at all times. The record must include the following particulars:

Pharmacy must maintain an individual record of administration including: individual’s name, address, telephone number, date of birth, allergies and gender; vaccine administered, the manufacturer, expiration date, lot number, site of administration, and dose administered; date of original order and the date of administration(s); name and address of Authorizing physician, and the name and address of Pharmacist administering the dose, and 5 N.J.A.C. 13:39-4.21(g),(h) & (k) * A pharmacist may administer an influenza vaccine to a patient under ten (10) years of age pursuant to a prescription by an authorized prescriber, N.J. Stat. 45:14-63. 6 N.J.A.C. 13:35-6.26; N.J.Stat. 45:14-63. 7 N.J.A.C. 13:39-4.21(i) the immunization location, if different from Pharmacy practice site; and name and address of the individual’s primary care provider, if provided.
Individual Records. Pharmacist must ensure that an individual record of administration is maintained including: individual name, date, vaccine given, manufacturer, lot number, expiration date, and signature of person administering vaccine. Records must be maintained for five (5) years.6
Individual Records. Pharmacy must ensure that an individual record of administration is maintained for a minimum of two (2) years including: individual name, address, and date of birth; date and site of administration; vaccine given, manufacturer, lot number, expiration date; name and address of primary health care provider as identified by patient; name or identifiable initials of person administering vaccine; documentation of informed consent; and the nature of any adverse reaction and any resulting notifications.11
Individual Records. 7 Pharmacy must maintain an individual record of administration including: individual name, address, allergies, and date of birth; date of administration; vaccine name, manufacturer, dose, lot number and expiration date; vaccine information statement provided; site and route of administration; name or initials of Pharmacist; the Pharmacy’s address; and any adverse reactions. Pharmacy must maintain records for at least seven (7) years after administration of vaccine.
Individual Records. 13 Pharmacy must maintain an individual record of administration including: individual name, address, and date of birth; date, route and anatomic site of administration; name, dose, manufacturer, lot number, and expiration date of vaccine; name and address of individual’s primary care provider; name or identifiable initials of Pharmacist; and nature of adverse reaction and who was notified, if applicable.
Individual Records. 5 Records must be maintained for three (3) years on all immunizations administered and must include at least the following information: patient’s name, address, date of birth, gender, and allergy information; date; name, strength, dose, lot number, and expiration date of the immunization; route of administration; injection site; and positive identification of administering personnel and person providing informed consent to administer the immunization.

Pharmacy must maintain an individual record of administration including: individual name, address and date of birth; individual responses to screening questions; administration date, vaccine given, site of injection, dose, manufacturer, lot number, expiration date; signed informed consent form; date VIS was provided and version of VIS provided; and signature or identifiable initials of person administering vaccine.9 Pharmacy must maintain all required records in a manner that is uniformly maintained, readily retrievable, and available for inspection for a period of three (3) years.10 Immunization records must be maintained separately from other records at Pharmacy.11
