Information Gathering and Assessment. The care management team must collect, maintain, and update all relevant information to conduct needs-stratification and to develop the Comprehensive Care Plan including: 1) Reviewing the results of the member’s screening and conducting other assessments for specific conditions as needed. 2) Collecting and analyzing a comprehensive set of available data regarding the member’s medical and behavioral health history including data provided by the Department. 3) Collecting additional information about the member’s social determinants of health.
