Investigation and Investigative Report. The County Department/Agency shall immediately investigate breaches and security incidents involving PII. If the initial PIR was submitted incomplete and if new or updated information is available, submit an updated PIR to DHCS within seventy- two (72) hours of the discovery. The updated PIR shall include any other applicable information related to the breach or security incident known at that time.
