IRO Reports. Within 60 days following the end of each three-month period during the term of this IA, Xxxxxx shall provide to OIG a copy of the Quarterly Claims Review Report prepared by the IRO for each Quarterly Claims Review performed, along with Xxxxxx’x response and corrective action plan related to any recommendations made by the IRO in the Quarterly Claims Review Report. Each Quarterly Claims Review Report shall include the information specified in Appendix B to this IA.
IRO Reports. Within 60 days following the end of each three-month period during the term of this IA, LFAC shall provide to OIG a copy of the Quarterly Claims Review Report prepared by the IRO for each Quarterly Claims Review performed, along with LFAC’s response and corrective action plan related to any recommendations made by the IRO, including LFAC’s determination of whether the CMS overpayment rule requires the repayment of an extrapolated Overpayment (as defined in Appendix B), in the Quarterly Claims Review Report. Each Quarterly Claims Review Report shall include the information specified in Appendix B to this IA.

Xxxxxxxxxx shall also provide (1) a response and corrective action plan(s) related to any recommendations raised by the IRO(s) in the Quarterly Claims and Cardiac Procedures Review Report; and (2) a response and explanation related to all instances in which the IRO determined that a particular Paid Claim was not medically necessary for each Quarterly Claims and Cardiac Procedures Review Report. Each Quarterly Claims and Cardiac Procedures Review Report shall include the information specified in Appendix B to this IA.

Each Pharmacy Quarterly Claims Review Report and the Quarterly Drug Inventory Review Report shall include the information specified in Appendix B and Appendix C to this IA.
