Common use of Level I Assessment Clause in Contracts

Level I Assessment. A broad assessment tool that will be used to assess the Enrollee’s current health and functional needs within forty-five (45) calendar days of Enrollment. This assessment will serve as the basis for further assessment needs that may include Long Term Supports and Services (LTSS), Behavioral Health (BH), Intellectual or Developmental Disability (I/DD) and/or Substance Use Disorder (SUD).
