Common use of Life and Disability Insurance Clause in Contracts

Life and Disability Insurance. The Company shall, in accordance with the Company’s policies, reimburse or pay on behalf of the Executive for up to $15,000 and $10,000 per year for Life and Disability Insurance premiums, respectively, with such reimbursements made in the calendar year in which the expense is incurred.

Life and Disability Insurance. The Company shall, in accordance with the Company’s policies, shall reimburse or pay on behalf of the Executive for all premiums paid by the Executive on life insurance policies covering the Executive in amounts up to $15,000 800,000 and disability insurance policies covering the Executive in amounts up to $10,000 20,000 per year for Life and Disability Insurance premiums, respectively, with such reimbursements made in the calendar year in which the expense is incurredmonth.

The Company shall, in accordance with the Company’s policies, shall reimburse or pay on behalf of the Executive for the costs of life and long-term disability insurance up to $15,000 and $10,000 20,000 per year year, provided that the request for Life and Disability Insurance premiums, respectively, with such reimbursements made in the calendar year in which the expense reimbursement is incurredsupported by relevant documentation.
