Life and Accidental Death. Dismemberment Insurance METRO shall provide term life insurance in the amount of $25,000 and accidental death & dismemberment insurance in the amount of $50,000 for twenty-four (24) hour coverage. Supplemental coverages will be made available and the cost shall be paid by the Operator. Plan coverages shall be reduced by fifty percent (50%) upon attaining age 70.

Life and Accidental Death and Dismemberment (AD&D) Insurance The College agrees to pay the premium costs for life and Accidental Death and Dismemberment (AD&D) insurance with a responsible insurance carrier equivalent to two (2) times the base salary for each bargaining unit member. The bargaining unit member may purchase optional life insurance coverage at the maximum amount allowed by the plan by paying the additional cost and adhering to the terms and conditions of the life insurance plan.

The City shall provide group life and AD&D insurance coverage on regular, full-time and regular part-time employees in the face amount of $50,000. Each employee may designate the beneficiary on such coverage. Life and Accidental Death. DISMEMBERMENT (AD&D) INSURANCE: Group Life Insurance and Accidental Death & Dismemberment Insurance at a value of $50,000 is provided to employees working at least 30 hours per week, at no cost to the employee. Employees also have the option to voluntarily “Buy-Up” more life insurance coverage for themselves and purchase life insurance for their dependents. All Buy-Up coverage would be the sole responsibility of the employee. This insurance is convertible; at the time an employee leaves service, to a term life policy.
Life and Accidental Death. Dismemberment Insurance The County shall provide term life and accidental death and dismemberment insurance for all employees covered by this Agreement in an amount of $50,000. The County shall pay the full premium for the insurance. The insurance is effective for each employee at the start of the first month which follows the satisfactory completion of thirty (30) days of employment. In the event of lay-off, the County will pay the premium for one (1) month beyond the month in which the employee was laid off. In the event of absence due to illness, the County will pay the premium during such absence not to exceed one (1) year. In the event of absence due to a Workers Compensation paid illness or accident, the County will pay the premium for the duration of such absence or two (2) years, whichever is lesser.
