Accidental Death and Dismemberment Insurance. The plan provides accidental death and dismemberment insurance coverage in an amount equal to your basic group life insurance (two times your current annual salary). Coverage is provided 24 hours per day, anywhere in the world, for any accident resulting in death, dismemberment, paralysis, loss of use, or loss of speech or hearing. If you sustain an injury caused by an accident occurring while the policy is in force which results in one of the following losses, within 365 days of the accident, the benefit shown will be paid to you. In the case of accidental death, the benefit will be paid to the beneficiary you have named to receive your group life insurance benefits. Benefits are payable in accordance with the following schedule: · Life · Both Hands or Both Feet · Entire Sight of Both Eyes · One Hand and One Foot · One Hand and Entire Sight of One Eye · One Foot and Entire Sight of One Eye · Speech and Hearing in Both Ears · Use of Both Arms or Both Legs or Both Hands · Quadriplegia (total paralysis of both upper and lower limbs) · Paraplegia (total paralysis of both lower limbs) · Hemiplegia (total paralysis of upper and lower limbs of one side of the body) · One Arm or One Leg · Use of One Arm or One Leg · One Hand or One Foot · Entire Sight of One Eye · Speech or Hearing in Both Ears · Use of One Hand or One Foot · Thumb and Index Finger of One Hand · Four Fingers of One Hand
Accidental Death and Dismemberment Insurance. The City shall provide for all employees by contract through an insurer selected by the City, Accidental Death and Dismemberment Insurance which provides for two (2) times the employee’s annual salary rounded to the next higher $1,000, if not a multiple thereof, if the employee’s death is as a result of an accident. The City shall pay one hundred per cent (100%) of the premiums.
Accidental Death and Dismemberment Insurance. Insurance equivalent to two (2) times annual earnings to nearest one thousand dollars ($1,000.00) that is higher.
Accidental Death and Dismemberment Insurance. The Employer agrees to pay one hundred percent (100%) of the billed premiums for such coverage for a minimum two times (2x) annual salary.
Accidental Death and Dismemberment Insurance. The Employer shall offer a group AD&D insurance policy for each full-time employee at the employee’s cost.
Accidental Death and Dismemberment Insurance. The Employer agrees to check-off and remit premiums for a Voluntary Accidental Death and Dismemberment Insurance Plan and to provide any statistical data necessary for premium quotation. Such policy and the carrier shall be determined by the Union. The Employer agrees to provide application forms and details of this plan to new employees. Voluntary A.D.&D. benefits in multiples of $10,000 are available and paid for by the employee.
Accidental Death and Dismemberment Insurance. Full-time faculty members may elect to purchase additional optional insurance coverage known as the Accidental Death and Dismemberment Insurance. All premiums for such additional insurance will be paid by the faculty member through payroll deduction. Those wishing to purchase additional insurance must enroll for coverage within 60 days of their employment date or during a scheduled open enrollment.
Accidental Death and Dismemberment Insurance. The University will pay the entire cost of accidental death and dismemberment insurance for a policy amount equal to the amount provided to a Faculty member in Section 21.2.
Accidental Death and Dismemberment Insurance. The County shall provide an Accidental Death and Dismemberment policy for death or dismemberment for all full-time Peace Officer and Supervising Peace Officer Unit employees and part-time Peace Officer and Supervising Peace Officer Unit employees whose normal workweek consists of at least twenty (20) hours. The policy shall provide benefits for death or dismemberment occurring in the line of duty.
Accidental Death and Dismemberment Insurance. To the extent a Participant elects, any or all of the Benefit Dollars credited to his account shall be used to pay the Participant's share of the cost of coverage under the Employer's accidental death and dismemberment insurance plan.
