Common use of Life Insurance Protection Clause in Contracts

Life Insurance Protection. A $5,000.00 life insurance policy shall be provided each employee. Life insurance shall remain in full force and effect for employees on paid sick leave or during an approved unpaid leave of absence of less than thirty calendar days. a. Life insurance for eligible employees will become effective the first of the month following the employee’s date of hire. b. Responsibility for enrollment in the program rests with the employee. Employees who do not enroll during the period of time as outlined in this section will not be eligible to enroll until the next open enrollment period. c. Eligibility of insurance paid by the Employer is based on the acceptance of the written application by the insurance carrier.
