Sick Leave Reserve. The purpose of the sick leave bank is to offset the financial loss incurred by a teacher who is seriously ill or a seriously ill member of their immediate family and has used all of their regular sick leave. Immediate family includes employee's spouse, children, parents or sibling. To use the bank, teachers will apply to the Board of Education, and will furnish such documentation as is requested to inform the Board and recommendation committee of the serious nature of the illness. Documentation will include doctor’s statements and other specific information found in the form provided for such use.
1. The sick leave pool will be voluntary, with each teacher electing to participate donating one day at the beginning of the 2002-03 school year. Those eligible to participate will be certified teachers as defined by K.S.A. 72-5413, part c, who are under contract to USD #325, and who have exhausted their regular sick leave. Part-time teachers may donate 1/2 day to participate.
2. Only those teachers electing to donate a day will have access to the pool.
3. The Board will either match, day for day, the teacher donations up to 50 days, or the Board will fill the bank to 100 days if more than 50 teachers elect to participate. The size of the bank will be 100 days or the number of participating teachers with matching board contribution; whichever is greater. The bank will remain in tact at 100 days for each succeeding year with teachers and the board contributing days equally to reach the maximum each year. No teacher will be asked to donate more than 1 day per year even if the bank falls below 100 for that year. Teachers will donate days after the first year following a list compiled of all participants. Each teacher will be given a random number to make up the initial list. Donations will be made from lower number to higher numbers. New participants will donate 1 day when they join, regardless of the number of days in the bank at that time, and will be put at the bottom of the rotation list. Teachers will donate 1 day as needed to maintain the maximum number of days in the bank, if their number comes up in rotation in order to remain in the pool. The pool days contributed will be carried over each year. If the pool falls below 30 days during any year, the participants will be notified and would have the option of donating more days to be matched by the board for that year.

Sick Leave Reserve. Teachers on sick leave shall have charged against their accumulated sick leave reserve only those days of absence that occurs while work is in session.
Sick Leave Reserve. 1. When an employee’s sick leave has been exhausted, an employee may be granted the equivalent of up to thirty (30) additional leave days as available and approved from the sick leave reserve. The following guidelines will govern utilization of the sick leave reserve.
a. In order to be eligible to draw from the reserve, the driver/aide/ assistant must be eligible for sick leave and must have contributed to the sick leave reserve.
b. Each employee will be invited to contribute hours equivalent to one (1) of his/her accumulated sick leave days to the sick leave reserve. A consent form for the deduction will be sent to each employee. Additional hours shall be requested of employees only when the reserve is depleted to five hundred (500) hours or fewer. If the reserve is depleted to five hundred hours or fewer, all employees will be invited to contribute one (1) sick day to the sick leave reserve and must do so in order to remain eligible for the sick leave reserve benefit. An employee who has previously donated one (1) of his/her sick leave days during the school year when the request for additional hours is made will not have t donate again to remain eligible for the sick leave reserve during that school year.
c. The sick leave reserve shall be available only for major personal illnesses and injuries of ten (10) or more workdays and not on a daily basis. A certificate of injury or illness form from a licensed physician must accompany the request for use of the sick leave reserve. The request must be received in the Human Resources office (if possible) prior to the effective date of drawing from the sick leave reserve.
d. An employee may not draw in excess of thirty (30) days from the sick leave reserve in any one (1) school year.
e. An employee who has used the equivalent of thirty (30) sick leave days shall not draw additional hours from the sick leave bank in a subsequent year unless the employee has contributed the equivalent of at least one (1) additional day to the sick leave reserve for the subsequent school year.
f. An employee will not accrue sick leave while utilizing sick leave reserve.
2. In order to be eligible to draw from the sick leave reserve, the employee must present a doctor’s certificate of injury or illness to the Human Resources office with the request to use the sick leave reserve.

Sick Leave Reserve. Employees may accumulate up to a total of 480 hours of sick leave in a Sick Leave Reserve. The purpose of the Sick Leave Reserve is to provide an employee with an economic cushion to be used in the event of a major illness or lengthy absence for medical reasons. No Sick Leave or Sick Leave Reserve will be paid at termination. Upon the death of an employee, any unused sick leave shall be paid to the employee's beneficiary at the employee's base rate of pay.
Sick Leave Reserve. It is the policy of the Henryetta Board of Education to encourage and reward personnel who exercise particular care in the maintenance of their personal health and job attendance. In accordance with Oklahoma Statues, Title 70, Section 6-104, employees may accumulate sick leave days up to a maximum of 90 days as prescribed by local school board policy.
1. In accordance with Section 17 116-2, the total creditable service of a member who retires or terminates employment and elects a vested benefit shall include not to exceed 120 days of unused sick leave accumulated subsequent to August 1, 1959, during the member’s membership with the Teacher’s Retirement System.
2. To clarify the difference between the two types of statutory sick leave days, the Board has established a sick leave reserve for the purpose of accounting for unused sick leave days, which may be used for retirement purposes, subject to the approval of the Teacher’s Retirement System of Oklahoma. The term “unused sick leave days” is defined as those sick leave days for which and employee does not receive credit after the maximum 90 days of sick leave have been accumulated. Unused sick leave days shall be deposited in the sick leave reserve.
3. The central office will maintain records for the purpose of assisting each employee to account for unused sick leave days for retirement purposes. The central office will account for and certify unused sick leave days for each year for which an employee has exceeded 80 days of sick leave.
4. Upon retirement, accumulated sick leave days and the unused sick leave days in the sick leave reserve may be certified to the Teachers Retirement System of Oklahoma to add creditable service for retirement purposes at a rate of one (1) month credit for each twenty (20) days of accumulated sick leave, up to a total of one hundred twenty (120) days. However, the school district shall not certify more than one (1) day of unused sick leave in the retirement reserve for each day of accumulated sick leave for the purpose of adding creditable service for retirement purposes.
5. The maximum forty (40) days of unused sick leave in the sick leave reserve and eighty (80) days of accumulated sick leave may be counted as an additional year of creditable service toward retirement of the Teachers Retirement System of Oklahoma, provided that the total number of sick leave days are acceptable to the Teachers Retirement System.

Sick Leave Reserve. 1. The Board will make provision for a sick leave reserve to aid unit members who suffer prolonged illness and whose sick leave becomes exhausted during an extended period of illness. The intent of the sick reserve is to provide a safety net for those members who suffer a long term illness or injury. The reserve is not intended to provide salary continuity for short- term illness or injury nor is it intended to cover cosmetic or elective procedures. Wherever possible, the member shall schedule procedures or treatments during the summer or at other times that would minimize absence from work.
2. In order to be eligible to use the sick reserve, the member must be employed by the District for one year and must be suffering from a disabling illness or injury that prevents the member from performing the essential duties of the position.
3. Each unit member who chooses to participate will contribute one (1) sick day by filing a signed authorization statement with the personnel office within the month of September or within 30 days after effective date of employment. When the number of available days falls below 25, participants must re-contribute one (1) day to maintain membership. If the Reserve falls below 25 days, the Board will contribute 65 days. Unused days will carry over into the next school year.
4. Unit members may use sick leave reserve days upon the following terms and under the following conditions:
a. the unit member must be an employee of the District on active status;
b. the unit member must have contributed at least one (1) day of sick leave to the reserve and maintained membership by contributing each time the reserve fell below 25 days;
c. the unit member must have exhausted his or her regular paid sick leave;
d. the unit member must have been absent with a qualifying illness or injury for a number of consecutive days as determined by the following formula: {40 days} minus {(Number of accumulated personal leave days as of the start of the school year September 1) plus (ten divided by the number of years service).}
e. the unit member must submit verification of the medical condition by the physician subject to the approval of the supervising school physician;
f. if recurrence of the same illness requires additional absence, the requirement under 4d will be waived and the unit member may be eligible for additional days but not to exceed the maximum of 90 school days allowed per school year.
