Common use of Location of Service Delivery Clause in Contracts

Location of Service Delivery. SUAs will normally be conducted at the Contractor’s Willmar location but may occasionally occur at the county jail or contracted detoxification center. Contractor shall accept referrals for SUAs from Kandiyohi County Family Services and Kandiyohi County Community Corrections. A shared calendar shall be maintained with immediate interactive access by both Agency and Contractor. Releases of Information shall be signed in accordance with applicable confidentiality laws. The Contractor will assure four slots a week will be on the shared calendar. This agreement is under MS 254A.19subd 3(3) in that the county retains responsibility for making placement decisions and thus avoids any financial conflict of interest.
