Special Features of Eddy SeniorCare. Eddy SeniorCare (PACE) is a unique program involving Medicare and Medicaid ben- efits authorized by the New York State Department of Health and the federal Centers for Medicare and Medicaid Services. The PACE model of care is designed to provide an alternative to those who prefer not to reside in a nursing home, but whose medical problems make it impossible for them to stay at home without coordinated care from doctors, nurses, social workers, and other providers of care. If you choose to enroll in Eddy SeniorCare, you will be participating in a prepaid, du- ally capitated, comprehensive health care program designed to provide all the health and long term care services from one responsible program. The program provides co- ordination of all one's health needs and long term home care, through doctors, nurses, home health aides, therapists, and other members of Eddy SeniorCare's interdiscipli- nary team. When selecting Eddy SeniorCare, you agree to only receive services from our Provider Network. Eddy SeniorCare is the sole provider who guarantees access to services, but not to a specific provider. By managing all of the health and long term care services through an interdisciplinary team, problems are identified, preventive care promoted, and hopefully emergency room or hospital stays avoided.
