Long-Term Services and Supports Community Partners. (LTSS CPs) 1. The Contractor shall maintain ACO/MCO – CP Agreements with at least two LTSS CPs within each geographic area served by the Contractor, in EOHHS’ determination, as further specified by EOHHS; 2. With respect to LTSS CP-Identified Enrollees, the Contractor shall assign each LTSS CP- Identified Enrollee to an LTSS CP within thirty (30) calendar days of being notified of such Enrollee;
