Medical Telework. To be approved for a medical telework agreement, the employee must submit a physician-certified written statement that: (1) Provides a description of the diagnosis of the medical condition necessitating the telework arrangement. (2) Summarizes the prognosis, including the expected return-to-work date, and, as appropriate, discusses medical management, including how the temporary medical condition might interrupt the employee's work schedule. (3) Lists restrictions that should be placed on the work performed at the alternative work location, if applicable. (4) States that the employee is able to perform the duties of the position at an alternative work location; and, (5) Describes the benefit to the employee’s medical condition from working at an alternative work location, or the reduction of health risks to other employees, if any, derived from this medical telework arrangement.
