Common use of Member Request For Medical Records Clause in Contracts

Member Request For Medical Records. County shall furnish a copy of a Member’s Medical Records to another treating or consulting Provider at no cost to the Member when such a transfer of Records; 7.8.1 Facilitates the continuity of that Member’s care or; 7.8.2 A Member is transferring from one Provider to another for treatment; or 7.8.3 A Member seeks to obtain a second opinion on the diagnosis or treatment of a medical condition; or 7.8.4 A Member's Records are needed to access Medi-Cal covered services not included in this Contract, including but not limited to mental health programs such as those provided by the Department of Developmental Services (DDS), CCS, and the Local Education Agency (LEA).
