Common use of Missed Appointment Fee Clause in Contracts

Missed Appointment Fee. If you cannot keep a scheduled medical appointment, please notify your health care professional’s office at least one (1) day prior to the appointment. If you fail to cancel your appointment, you may be responsible for the payment of an administrative fee for the missed appointment. The fee for a missed appointment at a Plan Medical Center is shown in the Summary of Services and Cost Shares section of the Appendix. This fee will not count toward your Copayment or Out-of-Pocket Maximum, if applicable.
