MUNICIPALITY. (hereinafter referred to as “the Municipality”)
MUNICIPALITY. A city, town, or village within the jurisdiction of the Central Racine County Health Department.
MUNICIPALITY. The person, corporation or other legal entity authorized by the Owner under this Agreement to attach its Facilities to Poles and the party responsible for compliance with applicable Law and Owner's rules, tariffs, policies, procedures and regulations regarding such accommodations. The term “Municipality” shall be construed to include a municipal Board of Education, public school department, or regional school district which provides education for the Municipality.
MUNICIPALITY. Any city, borough, incorporated town, township or county, or any municipal authority created by any of the foregoing. Operator. , or any permitted successors, assigns, or affiliates. Operator’s Facility. The Operator’s permitted facility that is offered to provide processing and/ or disposal services under this Agreement, located in (Township/Borough/City), County, Pennsylvania, or in (other state). Parent. Any corporation, now or at any time or times hereafter, owning or controlling (alone or with any other person) at least a majority of the issued and outstanding capital stock of the Operator.

If the Provider maintains more than one office, he or she will have to appear more than once in the list and file. Similarly, a physician or Provider with more than one specialty has to be listed for each specialty.);
