Notice of Pre-Disciplinary Hearing. In all discipline cases requiring a pre-disciplinary hearing, notice of such discipline shall be made in writing and served upon the unit member in person or by registered or certified mail. The notice shall include the following: 1. A statement of the proposed disciplinary action; 2. A statement of the charges from Section 20.1.6 of this Article upon which the proposed disciplinary action is based; 3. A statement of the facts and evidence upon which the proposed disciplinary action is based; 4. A statement of the unit member’s right to review or receive copies of any and all supporting documents or evidence related to the alleged misconduct upon which the proposed disciplinary action is based; 5. A statement of the unit member's right to respond orally or in writing or both; 6. A statement of the unit member’s right to have representation at the pre- disciplinary hearing. 7. A statement that the unit member may be eligible for the Constructive Action Program which, if approved, will cause the original implemented discipline to be rescinded and held in abeyance.
