Notice to Community. Within thirty (30) calendar days of the Effective Date of this Agreement, SFHMC shall provide notice of the availability of auxiliary aids and services free of charge. (A Model Notice to the Community has been provided at Attachment C for your reference.). The Notice shall be conspicuously posted and maintained in the lobby and admitting areas and wherever a Patient's Xxxx of Rights is required by law to be posted. The Notice shall be printed in bold font, with a character height for all words of 5/8 inch to 2 inches on a contrasting white background and will include the international symbol for “interpreters” and “TTYs” and the following statement, or language to the following effect: To ensure effective communication with patients, their family members and companions who are deaf or hard of hearing, we provide auxiliary aids and services free of charge. Sign language and oral interpreters, TTY’s, video remote interpreting (VRI), assisted listening devices, and/or other auxiliary aids and services are available free of charge to patients and companions who need them for effective communication. For assistance, please contact any AFAC Personnel or the Information Office at _____________(voice/TTY), room ______. These signs will include the international symbols for “interpreters." Additionally, AFAC will provide a notice containing the language found above to each patient when they check in for appointments.

Xxxxx shall continue to use its existing signs (see Exhibit B) and maintain them at prominent locations in the ECC including in the Triage, Registration, and Waiting Areas. Xxxxx will include in all future printings of its Patient Xxxx of Rights (or equivalent) and all similar publications a statement to the following effect: To ensure effective communication with Patients and their Companions who are deaf or hard-of-hearing, we provide appropriate auxiliary aids and services, such as sign language and oral interpreters, video remote interpreting services, TTYs, note takers, written materials, telephone handset amplifiers, , telephones compatible with hearing aids, televisions with captioning or closed caption decoders, and open and closed captioning of most hospital programs, will be provided free of charge when appropriate.
