Notification of Data Breach. The CONTRACTOR shall notify the One thousand ($1,000) dollars per BUSINESS DAY for which the standard is not met DEPARTMENT Program Manager and Privacy Officer within one (1) BUSINESS DAY of discovering that the PHI and/or PII of one (1) or more PARTICIPANTS may have been breached, or has been breached. The CONTRACTOR is required to report using the form provided by the DEPARTMENT. (See Department Terms and Conditions.)
