ID Cards. The MCOP shall mail ID cards to each member via a method that will ensure receipt no earlier than 15 calendar days prior to the member’s effective date of coverage and no later than one calendar day prior to the member’s effective date of coverage.
ID Cards. The Contractor shall issue an Identification Card to all new enrollees within ten (10) calendar days following receipt of the weekly enrollment file specified above but no later than seven (7) calendar days after the effective date of enrollment.

Failure to produce these sheets on request will result in the offending Club being fined £10.00, for the first offence, the second offence will result in a further £20.00 fine, plus the game shall be viewed as null and void with the points being awarded to the opposing team (where applicable).
ID Cards. Adjunct faculty members will be issued ID cards that, so long as they are members of the bargaining unit, will provide them with library, fitness center and recreational access on the same terms as full time faculty. They will also continue to have email access while a member of the bargaining unit.
ID Cards. All full-time and part-time students must have a valid UC student ID card. The card is the official College identification for students and should be carried at all times. Students must present their UC ID to any College official (e.g., safety officers, Student Living and College Engagement personnel, etc.) upon request.

ID Cards. All registered Summer Programs students taking courses on the UMass Amherst campus will receive Summer Programs photo identification (ID) cards. ID cards allow students secure access to the following Summer Programs resources and facilities including their Residence Hall, Dining Commons, University Health Services (residential students only); libraries, classroom buildings, computer and athletic facilities. Access to these facilities and services expires at the end of the summer session. Access to these facilities is a privilege; any students found engaging in behavior that may be deemed disrespectful or destructive will face disciplinary action which may include limitation of access, restitution for property damage and/or dismissal. No student may allow any other person to use his or her ID card for any purpose. Students should carry their ID cards with them at all times and are responsible for keeping their ID cards safe and in good working order. Lost summer programs ID cards should be reported immediately to the Residence Life Director’s office and the Summer Programs office. Once notified, the Summer Program s Office will issue a request for a replacement card, which students must then bring to UMass Amherst University UCard Office where a new card will be issued. The UCard Office is open Monday through Friday, 9 am to 5 pm. The replacement fee will be waived for stolen cards if the student provides a copy of a police report.
ID Cards. The Sheriff will issue laminated identification cards to all Employees which will contain at least the following: The Employee’s picture, the Employee’s job title, and the card’s date of expiration. It shall be printed in a manner which shall be easy to read.
