ID Cards. The MCOP shall mail ID cards to each member via a method that will ensure receipt no earlier than 15 calendar days prior to the member’s effective date of coverage and no later than one calendar day prior to the member’s effective date of coverage.
1. The MCOP will meet the timeliness requirement for mailing ID cards to members who select or change MCOPs, or change Medicaid-only or dual benefit status within the 5 business days prior to the end of the month, if the ID cards are mailed within:
ID Cards. The Contractor shall issue an Identification Card to all new enrollees within ten (10) calendar days following receipt of the weekly enrollment file specified above but no later than seven (7) calendar days after the effective date of enrollment.
ID Cards. Health Plan shall issue identification cards to Members and instruct them to present their cards to providers when seeking health care items and services.
ID Cards. Each Club is responsible for producing an A4 card showing pictures and details of their players, which shall be ready for viewing on request of an opposing Manager or any member of the Management Committee. Failure to produce these sheets on request will result in the offending Club being fined £10.00, for the first offence, the second offence will result in a further £20.00 fine, plus the game shall be viewed as null and void with the points being awarded to the opposing team (where applicable).
ID Cards. Adjunct faculty members will be issued ID cards that, so long as they are members of the bargaining unit, will provide them with library, fitness center and recreational access on the same terms as full time faculty. They will also continue to have email access while a member of the bargaining unit.
ID Cards. All full-time and part-time students must have a valid UC student ID card. The card is the official College identification for students and should be carried at all times. Students must present their UC ID to any College official (e.g., safety officers, Student Living and College Engagement personnel, etc.) upon request.
ID Cards. WPCRC ID cards are required for all MSCR participants. For participants who do not have a WPCRC ID card, MSCR staff will send appropriate registration forms to those participants with their class confirmations. Participants needing ID cards may obtain ID cards at WPCRC any time before the second class meeting. Class lists will be available at the WPCRC front desk on the first night of MSCR classes so that new patrons may be admitted and/or obtain their ID card on the first night of class. MSCR will be responsible for all MSCR registrations, fee collection, deposits and accounting as per MMSD policies. City staff shall have primary responsibility for the issuance and collection of fees for ID cards and for checking all patrons, including MSCR class participants, for ID cards at the front desk.
ID Cards. The CONTRACTOR shall issue ID cards within five (5) BUSINESS DAYS of the generation date of the enrollment file containing the addition or enrollment change, except as noted in item 4) below. (See Section 205.A)
ID Cards. (a) Upon Sponsor’s request, ESI will produce and send prescription drug ID cards for receipt by members on or before December 15 of each contract year at no additional cost.
(b) ESI will produce and send prescription drug ID cards for distribution to new Members at no additional cost.
(c) If new ID cards are required as a result of ESI’s error or ESI-initiated changes, ESI will maintain the responsibility to reproduce ID cards at no additional cost (including priority shipping).
ID Cards. FCHN shall require its Payors to provide each Participant with a membership identification card displaying the First Choice Health or Healthcare Direct name or logo, the Participant’s name and identifier, group name and/or number, telephone number to confirm eligibility and benefit verification, any applicable co-pay due at time of service, and utilization management vendor name and telephone number to confirm necessary pre-authorization for services. Provider is obligated to accept any individual as a FCHN Participant:
a) when the First Choice Health or Healthcare Direct logo appears on the individual's membership identification card,
b) where Payor is identified as accessing FCHN PPO Network in the FCHN tools, and/or
c) in cases where the Participant of a FCHN Payor has an Emergency Medical Condition and/or requires Urgent Services and is traveling or out-of-area and does not have a FCHN or Healthcare Direct logo.
