Common use of Operations Plan Clause in Contracts

Operations Plan. The Grantee has an HHSC approved operations plan and work plan that outlines goals, objectives, tasks, and timelines for each service outlined in the contract. The Grantee shall submit a plan of operation to HHSC within thirty (30) business days after execution of the contract. The Grantee shall submit to HHSC for approval on-going plan updates and changes at least thirty (30) business days before a change is to become effective. The operations plan shall include, but not be limited to, the following: a. Comprehensive description and delivery method for services provided to clients, and work plan on how coverage will be made available to the SNAP-Ed target audience in the State; b. Comprehensive description of how all geographical areas and high need but hard to reach populations will be served; c. Comprehensive description of strategies for evaluating the program and managing customer satisfaction; d. Comprehensive description of Xxxxxxx’s policies and procedures for each of the services provided by this grant; e. Hours of operation and the setting in which the services are provided by Grantee and network organizations; f. Job titles and contact information for administrative staff and their responsibilities related to fulfilling contractual obligations; g. Number and job classification of staff delivering services; h. Qualifications and competencies of each staff member paid from this grant; and i. Assurances that staff in the SNAP-Ed program met the required qualifications set forth within
