Optional Critical Illness. The Employer w ill offer the Optional Critical Illness plan w ith coverage in units of $5,000 up to $150,000 for employees and eligible spouse. There w ill also be an optional f lat amount of $10,000 for dependent children. Approval for coverage is at the sole discret ion of the insurance carrier and medical evidence may be required. Rates for Optional Critical Illness insurance are est ablished by the insurance carrier. Premiums w ill be 100% paid by the employee.
