Out of Classification Form. Written requests for out of classification pay from the employee shall be made on Employee Request for Out of Classification Pay form, attached as part of this Agreement in Appendix C. This form shall be distributed by the supervisor as directed on said form at the time the request is completed and once a determination is made by the supervisor/manager.
