Application Form. To apply for a pole attachment or conduit occupancy license under this Agreement, Applicant shall submit to SWBT two signed copies of the appropriate application forms. SWBT represents that the forms specified in subsections (a)-(b) are forms in use prior to the effective date of this Agreement and that SWBT is in the process of revising such forms to conform to the provisions of this Agreement and to streamline the application process. The parties therefore agree that the forms specified in subsections (a) and (b) will be interim forms only. SWBT reserves the right to change the format and content of these forms upon 60 days written notice to Applicant.
(a) To apply for a pole attachment license, Applicant shall submit to SWBT two signed copies of SWBT’s Form SW-9434 (“Access Application and Make- Ready Authorization”) together with completed Form SW-9433 (“Pole Attachments”). An application for a pole attachment license will not be complete or subject to processing by SWBT until these forms have been submitted to SWBT; provided, however, that such forms will be deemed to be substantially complete if they contain the information specified in subsections (c)-(h) below, as applicable. Copies of Forms SW-9433 and SW-9434 are attached to this Agreement as parts of APPENDIX III.
(b) To apply for a conduit occupancy license, Applicant shall submit to SWBT two signed copies of SWBT’s Form SW-9434 (“Access Application and Make-Ready Authorization”) together with completed Form SW-9435 (“Conduit Occupancy”). An application for a conduit occupancy license will not be complete or subject to processing by SWBT until these forms have been submitted to SWBT; provided, however, that such forms will be deemed to be substantially complete if they contain the information specified in subsections (c)-(h) below, as applicable. Copies of Forms SW-9434 and SW-9435 are attached to this Agreement as parts of APPENDIX III.
(c) Each application for a license under this Agreement shall include, at a minimum, the following information:
(1) the poles, ducts, and conduits (including all manholes) along Applicant’s proposed route to or within which Applicant desires to attach or place its facilities;
(2) a description of the facilities to be attached to SWBT’s poles and a description of the facilities to be placed within each component of SWBT’s conduit system (including but not limited to ducts, conduits, manholes, and handholes) along the proposed route;
(3) for poles, the proposed p...
Application Form. By electronically signing or submitting the Application Form you:
(a) acknowledge to us that you have received or downloaded, and read and understood this Client Agreement, our current PDS and FSG; and
(b) agree that we will provide our products and services to you on the terms and conditions of the Agreements.
Application Form. The applicant can secure application forms from the principal's office or from the Personnel office. The application shall be filed with the appropriate administrator.

Application Form. By submitting the completed Application Form to us, you:
2.9.1 acknowledge to us that you have received or downloaded, and read and understood this Client Agreement, our current PDS and FSG; and
2.9.2 agree that we will provide our products and services to you on the terms and conditions of the Agreements.
Application Form. You, by signing or submitting electronically the Application form when you are applying to become our client:
(i) acknowledge to us that you have read, accept, understood and agree to this Agreement and the other documents stated in clause 1.1 and their amendments thereof;
(ii) agree that we will provide our products and services to you on the terms and conditions of this Agreement.
Application Form. You understand that the Account Application is part of this agreement and that by signing on the last page of the application, you are agreeing to all of the terms and conditions in this agreement. You agree to promptly notify LPL in the event that your country of residence or citizenship status changes, and you acknowledge and agree that such notification may result in termination of your account by LPL if LPL does not service accounts in the new jurisdiction. In addition, if you are a sponsor or other responsible fiduciary of a retirement plan subject to the Employee Retirement Income Security Act of 1974 (“ERISA”), you are acknowledging receipt of the ERISA 408(b)(2) disclosures contained at the end of this Agreement. You must complete in full the application and you acknowledge the accuracy of its contents.
Application Form. 5.1. Pursuant to submitting the online Application Form, the Client makes the following warranties, namely: -
5.1.1. That the information submitted is true and correct; and,
5.1.2. That he has read and understood the terms and the Risk Disclosure Statement forming part of the Agreement.
5.2. In addition, the Client acknowledges and agrees to the following, namely: -
5.2.1. That he is bound to the terms and conditions of this Agreement;
5.2.2. That the Client consents to the sharing of any necessary Client Data, or relevant information, by RocketX to a third-party for verification purposes only, as provided for in terms of FICA;
5.2.3. if RocketX, as an accountable institution, is unable to adhere to its obligations in terms of FICA, including –
5.2.3.1. Establishing or verifying the identity of a Client, or obtaining relevant information, or information contemplated in section 21A of the FICA; or
5.2.3.2. Conduct ongoing due diligence as contemplated in section 21C of FICA, then RocketX –
5.2.3.3. May not establish or conclude a business relationship, or a single transaction with the Client, in the course of a business relationship,
5.2.3.4. Or perform any action to give effect to a single transaction; and,
5.2.3.5. Must terminate the relationship in accordance with its Risk Management and Compliance Programme, or any existing business relationship with the Client, and consider submitting a report in terms of section 29 of the FICA.
5.3. RocketX has the discretion to request Client information from the Financial Service Provided and will be given access to Client information within 3 business days from its request.
5.4. Should RocketX be unable to conduct the customer ongoing due diligence as required in FICA, the Client Trading Accounts will be disabled, and a record thereof must be kept for no less than 5 (five) years.
5.5. Nevertheless, XxxxxxX retains a right to refuse to open a Client Account at its own discretion, if deemed necessary.
Application Form. A signed Application for New Electric Service form is required to become a member and/or receive service. This application allows the privileges of voting and participation in the Cooperative's activities.
Application Form. An application for additional temporary income shall be:
i) on a form approved by the Superintendent;
ii) where the Annuitant is a former member of a pension plan, accompanied by the written consent of the Spouse of the Annuitant; and
iii) submitted to the Trustee at the beginning of the Fiscal Year of the Fund, unless otherwise permitted by the Trustee.
