Should you wish for your child to have access to these medications, you must sign a waiver and leave a supply with the school clinician or office. The following information must be printed clearly on the medication container: • Student’s Name • Name of the medication • Dosage • Time the medication must be taken. Bring in only the amount of medication that is needed for a school day. In the case of prolonged need, send in the amount for a clearly specified period such as one week or one month. Extra medication will not be sent home with a student. All medication will be kept in a secure location in the clinic.
