Participant Provider Service Agreement Language. The ICA must ensure participants have a participant provider service agreement with all required providers for all agency provided services. The participant provider service agreement shall be in writing; shall include the provisions of this subsection; shall only include approved waiver services and supports paid for using IRIS funds; and shall include and comply with any general requirements of this contract that are appropriate to the service. All amendments to the provider agreements shall be in writing and signed and dated by both the provider and the participant.
