Patient Safety Plan. 4.12.6.1 The Contractor shall have a structured Patient Safety Plan to address concerns or complaints regarding clinical care. This plan must include written policies and procedures for processing of Member complaints regarding the care they received. Such policies and procedures shall include: 4.12.6.1.1 A system of classifying complaints according to severity; 4.12.6.1.2 A review by the Medical Director and a mechanism for determining which incidents will be forwarded to Peer Review and Credentials Committees; and 4.12.6.1.3 A summary of incident(s), including the final disposition, included in the Provider profile. 4.12.6.2 The Contractor shall submit the Patient Safety Plan to DCH for review and approval within ninety (90) Calendar Days of the Contract Award.
