Patient Verification. The Parties shall develop a system to verify patient eligibility, as defined by HRSA guidelines. The system shall be subject to modification in the event of change in such guidelines. The Parties agree that they will not resell or transfer a 340B Drug to an individual who is not a Covered Entity Patient. Covered Entity understands that it may be removed from the 340B Program if it participates in drug diversion.

Patient Verification. The Parties will develop detailed processes for identifying Covered Entity Patients and ensuring that only Covered Entity Patients receive Covered Entity’s 340B Drugs.
Patient Verification. You agree to be solely responsible for verifying the identity, age, and authenticity of the patients who identify themselves to you and with whom you or your Authorized Users communicate through the RelayHealth Services. SORACOM shall not bear any responsibility in relation to dealing with such matters.
