Common use of Person-Centered Service Plans Clause in Contracts

Person-Centered Service Plans. The FIDA Plan shall assemble an IDT, with a Care Manager, for each Participant; and, the IDT, in conjunction with the Participant, will develop a comprehensive, written PCSP for each Participant. Within thirty (30) days of the FIDA Plan conducting a Comprehensive Assessment, a PCSP will be completed for each Participant by the Participant’s IDT. For Comprehensive Reassessments, a PCSP will be reviewed and revised, if deemed necessary by the results of the Comprehensive Reassessment, within thirty (30) days of the Comprehensive Reassessment. Every Participant must have a PCSP, unless the Participant refuses and such refusal is documented. The PCSP must include all elements outlined in the IDT Policy.
